A 55-year-old diabetic woman presented with subacute severe right pelvis and anterior thigh pain that was followed by weakness of bilateral quadriceps. Two months later, she experienced weakness of the hip flexors. She had no ankle or knee reflexes. Lumbar spine MRI showed mild degenerative disease of the spines. Her diabetes mellitus was well controlled.
